Care Coordination
ChristieCare provides support and consultation to help navigate children through systems of care. Our Care Coordination team works with families, local communities, courts, hospitals, mental health providers, school districts, cultural programs, employers, spiritual programs and others to ensure that children move seamlessly among living environments and service providers. ChristieCare staff help maintain continuity of treatment so that children attain their educational and vocational competencies, as well as their emotional and behavioral competencies.
ChristieCare currently possesses the capacity to provide Care Coordination in the following Oregon counties: Clackamas, Douglas, Lane, Lincoln, Linn, Marion, Multnomah, Polk, Tillamook, Washington and Yamhill.
